Privacy notice
ScholaGuard supports authorized school-health and campus-safety reporting in General Trias. Learner identity, health information, and incident narratives must be handled with care under the Philippine Data Privacy Act of 2012.
Information processed
Reports may contain learner names, birthdate, sex, school, optional LRN and guardian details, symptoms, immunization events, injuries, and safety incidents. Staff account information and safe request metadata support access control and accountability. Enter only information needed for the event.
Purpose and access
Information supports school-clinic care, authorized CESU investigation, DepEd oversight, referrals, and reporting. Access depends on role and school assignment; guardian details are restricted. Exports must be shared only with authorized recipients through approved channels.
Protection and retention
Directly identifying learner fields are encrypted in storage. Reports link to learner identifiers, and access is authenticated and audited. Authorized institutions must define retention, archival, and secure disposal schedules before deployment. Downloaded reports remain the responsibility of the receiving institution.
Automated assistance
Narrative classification and severity suggestions assist staff and require human review. Weekly school-level alerts compare report counts with recent reporting history. These outputs are advisory; they do not establish a diagnosis or predict an individual learner’s risk.
Your rights and contact
Data subjects may request information about processing, access or correction, and other applicable rights through their school’s designated privacy contact or the responsible CESU or DepEd office. Requests are subject to lawful obligations and appropriate identity verification. Privacy complaints may also be raised with the National Privacy Commission.
Institutional responsibility
The responsible institution must establish the applicable legal basis for sensitive personal information and provide any required learner or guardian notices and permissions. Reading this notice or accepting staff terms is not a substitute for that legal basis. Research evaluation should use synthetic or appropriately approved data.
